GLP-1 & Muscle Preservation

When losing weight with Ozempic, Mounjaro, and similar GLP-1 drugs, what you lose isn't just fat—it's muscle too. Protect your muscles, bones, and strength with resistance-focused reformer pilates under physiotherapist guidance.

Ozempic (semaglutide), Mounjaro (tirzepatide), Wegovy, and similar GLP-1 receptor agonist drugs significantly suppress appetite and provide substantial weight loss. However, this weight loss has an often-overlooked cost: research shows that approximately 25–40% of weight lost during GLP-1 treatment may come from lean muscle mass.

Muscle loss affects not only appearance but also metabolism, bone health, functional strength, and post-treatment weight control. For this reason, resistance exercise during GLP-1 treatment is not an option, it's a requirement.

Why Reformer Pilates?

Reformer pilates with adjustable spring resistance works all the body's major muscle groups in a controlled and joint-friendly manner. There are several reasons why it's ideal for individuals using GLP-1: it creates mechanical loading that stimulates muscle protein synthesis even in a calorie deficit; resistance training can be performed without excessive joint stress; progressing under physiotherapist supervision allows for load dosing tailored to individual condition and medication tolerance.

Program Scope

  • Muscle Mass Preservation: Progressive resistance work targeting all major muscle groups
  • Bone Health Support: Loading training that supports bone mineral density during rapid weight loss periods
  • Body Composition: Maintaining muscle tone while reducing fat percentage
  • Postural Awareness: Re-balancing posture as body weight changes
  • Energy Management: Individual adjustment of intensity during medication-related fatigue periods

Important Note

FiT48 does not offer a weight loss program. This program is designed to preserve muscle mass and bone health during GLP-1 treatment, improve body composition, and build a strong body structure. Your medication therapy and weight management remain under your doctor's supervision; this program complements your medical treatment and does not replace it.

What Pilates Gives You During GLP-1 Treatment

01

Muscle Mass Preservation

Resistance exercise stimulates protein synthesis even in a calorie deficit, supporting muscle preservation.

02

Bone Density Support

Loading training that supports bone mineral density during rapid weight loss periods.

03

Better Body Composition

More muscle, less fat at the same weight — a healthier transformation in appearance and function.

04

Metabolic Support

Muscle mass determines metabolic rate. By preserving muscle while losing weight, your metabolism stays stronger afterward.

05

Functional Strength

Preservation of strength needed for daily life activities, climbing stairs, carrying groceries, and other functional tasks.

06

Sustainable Habit

An exercise routine you can maintain after treatment ends — the foundation of long-term health.

How to Start the Program

  1. Health Assessment

Your current condition, the medication and dosage you're taking, your musculoskeletal status, and your goals are evaluated by our physiotherapist. Your medication tolerance and energy levels are reflected in the program design.

  2. Personalized Program Design

Intensity, load, and exercise selection are determined based on the stage of your GLP-1 treatment and your individual condition. Initial sessions begin at low intensity and progress gradually.

  3. Regular Follow-up and Adaptation

The program is updated as your medication dosage changes or your condition improves. Re-evaluation is performed every 4 weeks.

Frequently Asked Questions

Can I do pilates while using Ozempic or Mounjaro?

Yes. Resistance exercise while using GLP-1 drugs is not only possible but also necessary to prevent muscle loss. With physiotherapist assessment, you'll start at an intensity appropriate for your medication timeline and current condition. If side effects like nausea or fatigue are significant, session planning is adjusted accordingly.

Why do GLP-1 drugs cause muscle loss?

GLP-1 receptor agonists like Ozempic (semaglutide) and Mounjaro (tirzepatide) reduce appetite, creating a calorie deficit. This deficit is met not only from fat tissue but significantly from muscle mass. Research shows that approximately 25–40% of weight loss during GLP-1 treatment may come from lean muscle mass.

Does this program make me lose weight?

This program offered by FiT48 is not a weight loss program. Our goal is to preserve muscle mass and bone health during GLP-1 treatment, improve body composition, and build a strong body structure. Weight management is in the domain of your medication therapy and doctor.

Will I regain weight when I stop taking the medication?

Weight regain after treatment ends is well-documented. However, the exercise habits gained during treatment and preserved muscle mass make both maintaining a higher metabolism and managing the post-treatment period easier.

How many sessions per week are recommended?

2–3 sessions per week is ideal. Since individuals using GLP-1 may experience energy fluctuations, the number of sessions and intensity are individually adjusted. In the first months, 2 sessions per week is sufficient.
